Federal FFCRA leave expires - New form is available

A new form is available to employees needing to take leave for mandatory or precautionary quarantine. The New York State COVID-19 Paid Sick Leave Request Form has replaced the Families First Coronavirus Response Act (FFCRA) form, which expired on December 31, 2020. The new form can be accessed here https://www.nassauboces.org/nyscovidleaveform. All employees who have a Covid-19-related inquiry should contact covid19leave@nasboces.org.

Requirements to return to work after travel

The CDC is recommending against travel. If you have traveled to one of the locations restricted by the New York State Travel Advisory, you must complete the following steps to return to in-person work:

  1. Complete the Traveler Health Form;
  2. Before you depart the state or country that you are visiting, take a COVID-19 test within three days of leaving to return to New York;
  3. In New York, quarantine for three days;
  4. On the fourth day you are in New York, take a COVID-19 test. This must happen on the fourth day.
  5. Send copies of both of your negative test results to covid19leave@nasboces.org

If you are not able to follow these steps, you will be required to quarantine for 14 days from your arrival back in New York. Please visit https://coronavirus.health.ny.gov/covid-19-travel-advisory to see the advisory.
